For upcoming 7th grade students, the following forms will need to be completed for their 7th grade year either through the Clearfield website or via Sapphire.
1. Dental form. PA requires all 7th grade students to have a dental exam. This can be done at school or by their private dentist.
A Scoliosis check is required in 7th grade. If you wish to NOT have your child checked for scoliosis by the school nurse, please submit a letter stating you wish to decline.

At www.clearfield.org, please go to the Clearfield Area Jr./Sr. HS and click on “Health Center.” Click on “7th and 11th grade Health Screenings forms.” You will need to complete the 7th grade form stating if your child will be getting a dental exam at school or one done by their private dentist.

Complete the annual online health update form. Needs to be done YEARLY.
Complete the annual online over-the-counter medication form for meds (ex: Tylenol, Ibuprofen, antiacids etc.) to be given at school. Needs to be completed YEARLY.
If your child will need to take medications prescribed by a doctor, please fill out the “Medication authorization form.” This would include inhalers, ADHD, anxiety meds, etc. *IMPORTANT- A DOCTOR’S ORDER REQUIRED FOR MEDS PRESCRIBED BY A DOCTOR TO GIVE AT SCHOOL*
If your child requires an Epi pen, please fill out the Epinephrine administration at school form. *Doctors order required for Epi pen*
